All students enrolled in degree-granting programs must provide
documentation of immunizations that are required by state law. Required
immunization forms are included with the student`s letter of admission.
Students who do not submit documentation of immunizations prior to
enrollment or during the first term of enrollment are not permitted to
register for the following term. Requests for medical or religious
exemptions must be made in writing.
Documentation of additional health requirements set by the College of Nursing policy includes
1. complete physical examination and laboratory tests
2. a health history
3. tuberculin screening yearly (a negative chest X-ray is required for a positive test)
4. evidence of immunization against Hepatitis B
5. tetanus/diphtheria booster received within the last 10 years
6. documentation of immunity to varicella, measles, mumps, and rubella
Forms for these requirements are sent by the College of Nursing but
must be returned within one month of admission to the program to the
Immunization Program at Hurtado Health Center, Rutgers, The State
University of New Jersey, 11 Bishop Place, New Brunswick, NJ
